*Enrollment in the protocol establishes the timeline for your care. If you choose to discontinue use of the protocol before the full 90 days, your statistics and data will not be continued or considered for Phase II of our protocol. ALL Monthly Progress Questionnaires must be completed as well to be considered for phase 2 below.
Phase II: Physician Partnership Protocol
Phase II: Physician Protocol is on a REFERRAL basis after 90 days of the enrolled protocol (Angel Pak, Angel Pak +, Warrior Pak, Warrior Pak +) and contingent upon the review of (3) Monthly Progress Questionnaires. A large portion of individuals show improvement/relief in symptoms without needing a referral to physicians, which will include, cost of lab work, patient enrollment and standard appointment charges.
